S.M.I.L.E. Healthcare Program 

Self Manage to Improve your Life Everyday (S.M.I.L.E.) Healthcare Program

The Leduc Beaumont Devon Primary Care Network has a team of healthcare professionals that can help people living with various chronic conditions such as diabetes, pre-diabetes, high blood pressure, high cholesterol and obesity. A smoking cessation program is also offered. Our team, which includes a Registered Nurse, Pharmacist, Dietitian and an Exercise Specialist, work with patients to help them learn strategies to improve their ability to manage their health.

This program is by referral from a PCN physician ONLY.

Did You Know?

During October, November and December, 2011, the S.M.I.L.E. Healthcare Team lost

5700 minutes

due to no-shows or last minute cancellations. This equates to 190 appointments unavailable to our patients.  As well, there were 71 class registration spots that were lost to no-shows or last minute cancelations.
